Application Form and Supporting Documents

To apply for a grant, please read through the specification and guidance below, the complete the application form.  Applications are to be submitted via email to LA6Communitygrants@gateshead.gov.uk.  The closing date for applications is Friday 10th December 2021 at 5pm.  

If you are attending one of our live webinars, it is strongly recommended you have a copy of the application form for your reference.

If you are successful in your application, Organisations who recieve this funding will be required to meet record keeping requirements in line with ESFA and ESF funding guidlines.  Support, including templates and regular one to one meetings, will be provided by the LA6 Team to help organisations comply with these requirements. 

This secton will contain all of the forms and documents organisations will require to evidence their project.  There are two paperwork journeys each project must complete, the Learner Journey and the Project Journey.

Learner Journey
For each participant of the project, you must complete:
  • Assessment and Enrolment Form
  • Individual Learning Plan
  • Timesheet
  • Progression form
You may also need to complete:
  • Self Declaration of Eligibility Form - if the participant is unable to provide evidence to show they are eligible to recieve this funding under ESF funding rules. 
  • Withdraw/Early Leaver/Transfer Form - if an enrolled learner leaves the project before completion. 
  • Other documentation and/or digital learner signatures to confirm learner is in reciept of support and to declare relevant information, should the learning be remote. 

Project Journey
For each project, you must complete:
  • Each month of delivery, you must complete and return a Project Monitoring and Risk Analysis Report.
At the end of your project, you must complete:
  • Self Assessment Report (SAR)
  • Recognising and Recording Progress and Achievement Report (RARPA)
  • Record of Financial Expenditure
  • Case Studies for 3 of your participants. 

All of the requirements above have templates and supporting guidance for completing them, and will be available to download from  Thursday 16th December 2021.
